FEES: (A table, tablecloth, 2 chairs and lunch for staff at the event is provided to all our exhibitors. No other paperwork, other than this form, is necessary.)

$1,000 GOLD LEVEL
Benefits include:
• Name and logo on our event flyers and all advertisements
• Name will be placed on program
• Name will be displayed as sponsor on the press release
• Name and logo displayed on event t-shirts
• Recognition at each workshop
• Table space at the health fair

$ 500 SILVER LEVEL
Benefits include:
• Name as Silver sponsor will be placed on program
• Name and logo will be on event t-shirts
• Table Space
